Hazel M. Johannsen, 80, of Madison, passed away.  Funeral services will be at 11:00 a.m. on Monday, September 5th, 2011 at St. Peter Lutheran Church of rural Madison, with burial in the church cemetery.
She is survived by her children:  Craig Johannsen of Winfred and Wanda Paul of Madison; a sister - Loretta of Owatonna, MN.
The Randall Funeral Home of Madison is in charge of arrangements.
